I tried the above web site to change address for health card. It works for my wife and my son, but not for myself(to be figured out).
I used the latest version code for updated card and the last 4 digits in the right bottom of landing paper IMM1000. (e.g. W999 444 555, using 4555).
Wish it helpful.
By the way, don't forget to change the address for your vehicle permit (Ownership) if it hasn't the same name as the Driver's licence.
