是呀,这就是你宝宝的出生证。上面不仅仅有“Statement of Live Birth" 的复印件（是不是后悔当初填写Statement of Live Birth的时候没有把字写得好看一点？：），你仔细看看，上面还有出生证的号码。

What is the difference between a Short Form (wallet-sized) and a Long Form?


A Short Form contains the following information:

Name (Surname and given name(s))
Date of birth
Certificate Number
Birthplace
Sex
Date of Registration
Registration number
Date issued



A Long Form is a certified copy of the birth registration and contains the following information:

Name (Surname and given name(s))
Date of birth
Birthplace (city, town, village, municipality)
Sex
Location of birth (hospital, birthing center, etc.,)
Parent's names
Parent's date of birth
Mother's mailing address at the time of birth
Duration of the pregnancy
Number of children born to this mother
Parent's birthplace
Weight of the child at birth
Type of birth (single, multiple, etc.,)
Attendant at the birth
Date of Registration
Registration number
Date issued


Do you need a certified copy (long form) of your birth certificate?

You may need a certified copy (long form) of a birth certificate if:

Your employer requires it
You are moving to another country
You are filling out certain citizenship documents

long form上有父母的名字,可以申请护照什麽的,Short Form 就不行了,但是方便携带.所以通常两个都要.
