My relatives just immigrated to Canada and they have a high-school student to enter Canada high-school, so now we are urgent to need a Routine Immunization Form to fill. Who can give me an empty Form for a high-school student. Or tell me how to obtain this Form (website or telephone number to contact an organization in Canada).

Go to the above website and check out the Family Receiption Centre document. Then call the number listed on there. They will tell you where to get the form. Good luck.
