If your child was born outside of Canada, Ontario Registrar General may not return your original birth certificate if you submit it. Of course, it must have an English translation. They will not issue a new certificate either. That's why you may want to submit a notarized birth certificate, which will be returned later.

It is my experience.
If you child has an english name in school, you can add his english name to the "Name to appear on citizenship certificate (supporting documentation required)" in Canadian citizenship application form.

There is no additional charge except the normal citizenship application fee.
*** Note: You need to provide the school transcript (with his english name) to Citizenship and Immigration Canada.
If the school transcript does not have your child's english name, you need to change his/her name by applying to Ontario Registrar General. (fee applies)
