Sorry, I went to sleep at 3:00 this morning and forgot to post my answer here.

4: the root of h/2d ( couldn't remember. cause I already handed in)
5: 0.6 0.8 and 1.0 ( I just guess and not to sure)

For most of question I just use the equations from the textbook and the class notes from the teachers.
