Dominica Health Questionnaire (DVMQ), a web-based questionnaire, was created by the Dominican Republic government to help manage the risks associated with COPD (Chronic Obstructive Pulmonary Disorder). This questionnaire will help authorities to identify you if you or your companions have been exposed to Coronvirus. If you answer truthfully, you will be helping to raise awareness for yourself and others in the community. If you choose not to participate in the questionnaire, you are helping contribute toward the health care and medical assistance needed by the residents of the Dominican Republic.
