需要把short form和long form的birth certificate都申请了么？

好像除了护照申请需要long form 外，没有提到专门需要short form的。所以打算就办long form好了。
Birth Certificate (Short Form, card-sized)

Birth Certificate (Long Form)
A Long Form document is a certified copy of the birth registration and contains the following information:
Name (Last Name and First Name(s))
Date of birth
Birthplace (city, town, village, municipality)
Sex
Location of birth (hospital, birthing centre, etc.)
Parents' names
Parents' dates of birth
Mother's mailing address at the time of birth
Duration of pregnancy
Number of children born to this mother
Parents' birthplaces
Weight of the child at birth
Type of birth (single, multiple, etc.)
Attendant at birth
Date of Registration
Registration Number
Date issued
A first-time Long Form Birth Certificate costs $35.00
The Long Form is 21.6 cm x 35.5 cm (8.5. in. X 14 in.).

没多想，当时就觉得就 25 块，办一个吧。 将来给孩子也算是个礼物，对不对啊
这个以后真需要也可以再申请的呀。
