Any ONE of the following Primary Documents:
-A copy of the valid passport or travel document

OR

- A copy of the passport or travel document held at the time the applicant became a permanent resident

OR

-A copy of the certificate of identity or travel document issued by the Minister of Foreign Affairs of Canada

我提供护照前两页复印件。我们新换的护照还没有中文盖章呢，省了翻译费。

Along with ANY ONE of the above documents,he/ she should produce

- A copy of any identity document issued outside Canada before he/she came to Canada

OR

-A statutory declaration signed by the applicant attesting to his/her identity and a statutory declaration also attesting to the applicant's identity signed by:

- a person who knew the applicant before he/she came to Canada, such as a family member

OR

- an official of an organization representing people from the applicant's country of nationality or former residence.

Any ONE of the following Secondary identity documents:

- A copy of the Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292)

OR

-A copy of the valid provincial driver's license, provincial photo-identity card or university or college student card

OR

- A copy of the most recent Income Tax Assessment, issued by Canada Revenue Agency

CIC recommends that the applicant submit a copy of the Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292).

- A copy of the applicant's birth certificate (showing the applicant's name, date of birth, place of birth and the names of the parents or adoptive parents) ,

OR

- Legal documentation proving guardianship, if the applicant has a legal guardian

Translation of documents
Any document that is not in English or French must be accompanied by:
* the English or French translation;
* a certified copy of the document; and
* a translator's declaration.

只有小孩的中国出生证要翻译认证。

然后寄驾照到她办公室
3天之内就会翻译并公证好并寄回Ottawa！！！
