Please help me with following registration form in French to be finished before I mail it to Quebec University in Hull to confirm my registration for my PMP program.(如下的：Sigle,Gr. Titre 这几个单词分别是什么意思?）

Could that be like this (not quite sure though):

Sigle: the Abbreviation course or program
Gr.: (groupe) the group or department it belongs to
Titre: the tittle or description of it

Is this "Sigle,Gr. Titre" 5 line form asking me to select courses I will take in this September semester?
Must I choose all of them? or can only choose some of them?

Can you scan the entire form and post it here, so that I can have a whole view instead of just guessing? And I think what mickyuqo said sounds pretty rational.
