Draft report into bus-train crash complete

The Canada Transportation Safety Board has completed a draft report into the 2013 bus-train crash that killed six people near Fallowfield station, including the OC Transpo driver.

The report has been sent to a list of reviewers for comment on its technical accuracy and will be returned to the TSB by Aug. 20 for updating.

There is no word on when it will be released publicly. First, the comments must be considered and, in some cases, incorporated into the report. “There is still a lot of process yet,” said CTSB spokesman John Cottreau.

The board is mum on the list of reviewers currently looking at the draft report, but the City of Ottawa is likely among them. City manager Kent Kirkpatrick wouldn’t comment on the status of the TSB investigation or whether they had received a copy of the draft report.

The crash during morning rush hour on Sept. 18, 2013, killed five passengers and the bus driver, Dave Woodard.

Investigators have already raised driver distraction and speeding as possible factors in the crash.

The TSB released an update on the case last September that revealed new details, including that Woodard seemed to have been briefly distracted just minutes before the crash by a small video screen in his cabin.

The TSB said bus drivers are told not to stare at the monitor while driving, but may have to if passengers on the upper deck are standing up. Drivers are then required to inform passengers they aren’t allowed to stand on the upper deck.

Seconds later, Woodard heard the screams of passengers and applied the brakes. Woodard was driving about seven kilometres an hour over the posted 60 km/h speed limit. The city has since reduced the speed limit for buses in that stretch of Transitway to 50 km/h and is reviewing the video screens in double deck buses.

The final report should have additional details about the factors that led to the crash and recommendations about how future incidents can be prevented.

At least 15 lawsuits have been filed against the city by passengers and families of passengers who were in the crash.
