Costco那张不能用了。我没有接受他们后来改变的卡。

如果你还有其他的amex卡，可以把Costco的那个卡cancel了，把余额转到其他的amex卡上，或者让他们寄支票。

总之，你好像得把那个卡cancel掉，才能取回钱。

You must have been sent Simply Cash Amex card around this time last year. You must have used the card for some purchases, therefore you have the cash back, it was 2% (5% for gas, grocery and restaurant during the first 90 days). Since you have a credit on your account, and you don't plan to use the card, you can log in to your Amex account, click on "email us" on the left, and send them a secure message, asking for the cheque to be mailed to you in that amount.
