白内障手术

我公公两年前做过，Dr. Poleski at Montreal 做的。印象中没有收费。术前滴眼的药水是医院提供的好像。也许我记错了。晶片也选择OHIP covered 的。总之没有难题的样子。做完后视力提高不再戴眼镜。是晶片有校正视力的作用吗？

然后我婆婆今年做的。 Dr。 Smith at Carling。 开始检查眼睛的时候要求收$ 120， 但后来跟进了一下又没声音了。然后药水要收费。晶片推荐用高级一点的， 叫AMO TECNIS，交了$140. (可能我老公记错了，140 是一只眼睛的价格， 或者她只有一只眼睛用高级晶片？另一只视网膜破裂过几乎瞎了) 做完视力提高，以前千余度近视，现在自估7百左右。刚做好还没配眼镜。

现在我爹要做, 医生是Dr。Marshall at Centerpointe.，其实他自己不觉得有问题。是年检时医生说成熟了可以做。只能听医生

有两个问题还在琢磨。

Prior to all cataract surgery the eyes need to be measured to determine the strength of the lens implant the doctor will use. Doctor Recommends all patients have both the Ascan and IOL Master. The IOL Master tends to be more accurate and the Ascan confirms the results. Ascan is covered by OHIP and IOL Master cost $220.

需要做IOL 吗？Ascan 是用超声波检查，IOL是用激光检查所以精度高。问题是选晶片强度只要粗估就行我猜，不需要很高精度我猜。 我公公可能没做，因为没记得有收费。我婆婆被要求交的$ 120是这个检查？是不是算了不就220吗。

晶片有两种选择，普通的OHIP covers， TECNIS$140一个. 好处是, can increase contrast sensitivity for some patients. What this mens is that in low light conditions, the patient may see some details in the shadows slightly more clearly. This could come in useful in situations such as driving at night. In brighter light situations there should not be difference. The cost of this upgraded lens is currently $140 per eye. 有必要吗。还有更高级的ＴＯＲＩＣ＄５５０　一只。当初我公公作连想都没想就用普通的。

普通OHIP covers 的就好，不要用 upgraded 的。也不要用较正视线的，平光的就好。别花钱买罪受。
眼睛是有适应性的。若有近视、或远视，可用眼镜纠正。

你的话有偏见，我们家老爷子2002年做的手术，放的镜片到现在都很好，他原来的视力，眼镜度数是2000多度，手术后再也不用戴眼镜了，享受不戴眼镜的快乐

1. Ontario government paid ophthalmologist $ 495 CAD per surgery. The surgery, is using laser, only takes 10 minutes to complete. Ophthalmologists think they are underpaid, hence, some will ask for extra money. Do remember Government pays salary of nurse, operation theater, hospital staffs, and CAD is net paid per 15 minutes work to eye doctor. And ophthalmologist usually doing 3 continuous hours of surgery in one morning, 6000 CAD net.
2. OHIP covers ULTRA sound measurement, usually ophthalmologist will ask patients to go for "better" lens,which is not covered by OHIP, and they claim that needs "LASER" eye measurement, which is not covered by OHIP, the cost ranging from 250 CAD to 350 CAD (any kick back??).
3. Expensive lens ? In Toronto, there is an ophthalmologist, the cheapest lens is 800 CAD, highest 2450 CAD per lens, plus other expenses, 350 CAD for laser eye measurement. (Professional ethics???)

如果度数比较明显，趁做白内障手术的时候放有度数的镜片进去是比较好的选择。眼睛这时是基本不能自动慢慢调节的。

白内障是晶状体长了层白膜，所以看不清，而眼睛度数会随时间改变，要不要趁做白内障手术的时放有度数的镜片，是好是坏纯属个人选择。我个人倾向两者分开。

不建议选择 2.
原因是事先计算的度数等手术完成过一段时间后都会有误差，这时如果再去配眼镜，由于两眼度数差别大，这个眼镜会不舒服，也可能会有复视的情况（左眼，右眼看到的同一物体图像会不一样大）。做成一只眼看远，一只眼看近的医生一般是比较老的概念，也会影响我们的距离，方位的判断能力。类似于卫星定位，多颗卫星才能准确定出方位和距离。

同意，我家老人就是一远一近，我发现时间长了没有适应，倒是看远近都是一只眼瞄准似的，最后还是配眼镜，不如都看远，戴老花镜简单。
