Section G: Statutory declaration in lieu of guarantor
Complete this section only if you are unable to provide a guarantor’s declaration.
You must declare this section before, and have it signed by, a qualified official who has the authority to administer an oath in Canada (a commissioner, notary public or person authorized by law to administer oaths in all matters in the Canadian jurisdiction in which it is subscribed). The qualified official must also sign the back of one of your photographs (the photograph must not be sealed or embossed).
You must present the official with photo-identity documents such as a passport, driver’s licence or provincial photo-identity card to prove your identity and signature. List the documents in the space given.
