Passport Application - If you are unable to provide a guarantor for your passport application, you can make a Statutory Declaration in Lieu of Guarantor, which the Commissioner will then administer. Occasionally, we will have blank passport applications and Statutory Declaration in Lieu of Guarantor forms for use by our clients. However, it is recommended that you obtain a copy of the passport application and Statutory Declaration in Lieu of Guarantor form from the passport office before your appointment. The passport application and Statutory Declaration in Lieu of Guarantor form should be completed, but not signed and dated before you visit our office.

Fees:
$37.50 (includes G.S.T.). The fee for commissioning a passport application includes the certification of photographs and certification of documents that support identity.
