HELP: what is Client ID in supplementary Identification form of Maple card

I am filling the supplementary identification form
i dont know what i am expected to fill in client ID,
The same question in the fee receipt too.
Shall i put my passport number or driver licenses
or SIN number? shall i provide the certified copy
too if i use any of them as my client ID?

Call CIC! They will give you a clinet ID.
