Applicants who wish to legally change their name must have a minimum of one first name and one last name. Please note: The Office of the Registrar General may be unable to register some symbols and reproduce them on certificates.

In your application to change your name, you will be advised of the detailed requirements for each type of change of name application, including:

All birth certificates and previous change of name certificates of each person whose name is to be changed.
A statement by a guarantor who knows the applicant and can confirm that the applicant has lived in Ontario for at least one year prior to the application date.
A marriage certificate, divorce certificate, annulment certificate, a death certificate of spouse or joint declaration of conjugal relationship.
Proof of notice to your spouse if you are married and are formally changing your name to another surname.
Proof of custody and written consent from all those who have custody if you are changing your child's name.
Proof of notice to every person who is lawfully entitled to have access to the child.
Written consent of the child, if the child is 12 to 18 years old.
The required fee.
Other documents may be required, depending on the circumstances. Please read all the instructions carefully.

To legally change an adult or child's name, visit the Change of Name Wizard. For information on other change of name applications, contact the Office of the Registrar General at 1-800-461-2156 or in Toronto at 416-325-8305. Please note that the Office of the Registrar General is experiencing very high call volumes and it is difficult to get through.

It takes 1 or 2 months to get application forms, and another 1 year for processing. === Super Slow ====
