Hello,
You have been invited to participate in a survey on firm innovation.
Innovations are critical to business success. This research examines
the success factors of firm innovations. Your completion of the
following survey will help us with this research effort. If you have
ever STARTED a company, please complete the survey, regardless of
the current status of the company. Time of completion is expected to
be only 20 to 25 minutes.
There are no foreseeable risks associated with this project, and
your participation in this study is completely voluntary. Your
survey responses will be strictly confidential and data from this
research will be reported only in the aggregate. You may also
withdraw from participation at any time should the questions make
you feel uncomfortable. Should you choose, you will also receive an
analysis report once we complete this research (by Jan. 31st 2006).
