急问： 公民申请时怎样出具小孩的出生证明？

如果出生证原件不是英语/法语的，要提供:复印件+翻译件+翻译人的affidavit. 这里的affidavit是指声明吗？任何人（家庭成员除外）翻译的都可以吗？

只出具出生公证可以吗？

出生公证可以(same as when you applied for immigration). If you want to provide a photo copy, you can ask your bank to certify as "the true copy" (bring the original with you and the bank clerk will do it for you.).

Yes, but it must be certified. This is only my understanding. You'd better check the document carefully to see if photo copy is OK. Or if it is even required.
