check the package from hospital
if you delivered baby in Gatineau hospital, should got a big evenlop including a form for child care benefit. This application form will have both national and uccb at the same time. I got that form in Fr so i just downloaded online. you can check ur package first.
for quebec, i don't think you need to apply since after you fill in the form for baby name, all info will be transfer to regie for quebec child benefit.
