请教：向雇主提出maternity leave的一些问题 (不是政府，而是向雇主），谢谢

我的预产期预计是明年1月30号。初步打算从今年11月1号开始maternity leave (到due date大概是13周)。

我知道现在还没到向政府申请benefit的时候，但请教以下几个问题:

1) 我现在就可以向雇主提交maternity leave的申请吗？也就是提前让公司知道我要在11月开始离职。
或者必须等到某个时候才能提交申请，比如离职前一个月？

2）请问哪位妈妈有这种书面申请的范文，我可以借鉴？谢谢

3）除了书面申请，还需要材料证明怀孕吗？比如ultrashound report?....

4）在提交申请的时候，其他还需要注意些什么？

my experience is that your company's HR has all the forms needed. I think you just need to fill these form and ask your manager's signiture on the form. that's it.

谢谢。因为是小公司，所以要自己申请填写。再问一个小问题：你们申请的时候是MATERNITY和PARENTAL LEAVE一起申请的吗？也就是"Leave request for period"这个时间段。是从11月1号到due date，还是到宝宝出生后的35周？谢谢

There seems to be a problem with your plan. Here is what I copy and paste from Service Canada website.

The mother can start collecting maternity benefits either up to 8 weeks before she is expected to give birth or at the week she gives birth. Maternity benefits can be collected within 17 weeks of the actual or expected week of birth, whichever is later.

谢谢提醒。这个规定我看到了的，知道maternity benefit是要从due date算起，最多8周前可以开始领。但我的打算是先多休息几个礼拜，再开始申请benefit。

顺便请教这样是否可行：

-- 从11月1日离职，没收入了
-- 从12月初开始申请maternity benefit，直到宝宝出生

政府允许申请maternity benefit之前的1个月左右时间赋闲在家吗？（这一月不是sickness leave）

此外，还是那个老问题，现在是否就可以书面向公司提交maternity leave的报告呢，还是要等到某个期限之内？谢谢

政府可能不太Care你什么时候工作，什么时候休假吧！你说的11.1离职，这个我估计主要看你的雇主同意不同意了。你这个算Leave without Pay，我一个朋友每年夏天都这样请假带孩子回国过暑假，一次就两个多月呢。她老板每次都特痛快地就同意了。

我听说的大部分人都只会提前2个星期休息，尽量把假期留在孩子出生后在家照顾孩子。

我觉得你可以先口头跟你的上司讨论一下，如果你们公司有HR，也可以问问。看看他们要求你写什么样的申请或者有没有表格要填写。各个公司这方面的规定不太一样吧？

你们公司没有现成的文件吗？
我们公司要求签下面的文件：
1。什么时候开始修产假，什么时候回来上班。
2。修产假期间的假期怎么办？bank起来还是pay out。
3。修产假期间的health & dental benefits怎么办？停止还是持续，全部还是一部分。
4。如果公司补贴钱，那要保证在修完产假后一段时间还在公司上班。
5。医生开的怀孕证明。
