To change Ontario plate and driver's license to Quebec ones, what is the document that SAAQ requires: Driver's some sort of record??? (Driver's Abstract???)

What is it exactly??? Went to the Ontario transportation office, they don't know and even SAAQ couldn't really tell the exact name???
