The subsidy interview
The assessment interview will take about 30 minutes. During that time, the case coordinator will determine whether you qualify for a subsidy by looking at:
Your family’s income as determined by Line 236 of the most recent Federal Notice of Assessment for each parent or total family income from the Canada Child Care Tax Benefit Notice or the Ontario Child Benefit Notice.
Each parent’s need for Child Care as determined by hours of work, school attendance, etc.
Please keep in mind that this list is only a guide to help you get ready for your interview. You may not need all of these, or you may be asked to bring in other papers as well. The papers are needed for all members of the family including the children.

Papers you will need
Checklist of Mandatory DocumentsIdentification - such as birth certificates, passports, Immigration documents or baptismal certificate.
Income – Federal Notice of Assessment, Canada Child Care Tax Benefit Notice or Ontario Child Benefit. This is mandatory for any determination of eligibility to happen and an interview will not be conducted unless the family has this information.
Students - school registration along with time schedules Ontario Student Assistance Program (OSAP) assessment breakdown.
Proof of hours of work – pay stubs or a letter from the employer stating hours of work.
New Canadians - original immigration papers.
Verification of address – mortgage document, transfer of land, lease or rent receipt.
