New Patient Registration
Steps to becoming a new patient!
1.Print off and complete the following Greenbelt medical health form.
Greenbelt FHT Medical Form
2.Complete Ministry of Health patient registration form. (available for completion at registration events)
3.Submit completed medical health form and Ministry of Health patient registration form at one of our registration events.
4.Once both forms are completed and submitted to the clinic, patients will be able to book appointments with their newly assigned family doctor.

Saturday 9am to 12noon
