There was no such thing as a "TN" visa for Canadians. (There
is a TN visa for Mexicans).
Make sure you understand what is a visa: a visa is a permit that allow you to proceed to the POE (Port of Entry) and apply for admission. In essence, visa is a permit, not an authorization.
Canadian normally do not need visa to proceed to a US POE, we can apply for TN work authorization there directly. Mexican on the other hand, must apply for TN at US Embassy/Consulate, so theirs is a visa.
Although recommended, Canadian do not need passport to apply for TN, birth certificate, citizenship card, or expired passport, anything that can prove citizenship are OK.
TN VISAS: Professionals Under NAFTA
Who qualifies for a "TN" visa?
The category "Professionals Under the North American Free Trade Agreement" is available only to citizens of Mexico and Canada. Under the North American Free Trade Agreement (NAFTA) a citizen of a NAFTA country may work in a professional occupation in another NAFTA country provided that 1) the profession is on the NAFTA list, 2) the alien possesses the specific criteria for that profession, 3) the prospective position requires someone in that professional capacity and 4) the alien is going to work for a U.S. employer. The spouse and unmarried, minor children of the principal alien are entitled to the derivative status, but they are unable to accept employment in the United States. Aliens entering under this classification are considered non-immigrants.
How can a "TN" visa be obtained?
The requirements for Canadians and Mexicans wishing to enter under this classification are not the same. Citizens of Canada must provide the following at the port of entry:
1. A request for "TN" status;
2. A copy of the applicant's college degree and employment records which establish qualification for the prospective job;
3. A letter from the alien's prospective U.S.-based employer offering him or her a job in the United States, which is included on the professional job series (NAFTA list); and
4. A fee of U.S. $50.00.
Canadian citizens are not required to obtain a visa, but instead receive "TN" status with the Bureau of Citizenship and Immigration Services in the Department of Homeland Security (BCIS) at the port of entry. The "TN" status will only be granted if the period of stay is temporary.
The requirements for Mexican citizens are as follows:
1. First, the prospective employer must file a labor condition application;

3. After the petition has been approved, the alien must apply for a non-immigrant visa at a U.S. Embassy or Consulate in Mexico.

Wrong. My roomate got rejected. Offer letter is not Enough. It must have the followings:
1 How long will the job last -- Please only put 1 year there, otherwise they will not admit you.
2 Details of job functions. They will compare them with your educations. For examples: Say something Using Java, OO design, then in your cource you have OO design etc... As long as your job description does not have Medical Operation and your degree is from Computer Sci, then you are most likely fine.

Offer letter does need to include duration of employment, details of job functtion and qualification. So you can call this as a specialized offer letter. I always make sure those details are in there before heading to the boarder. Appology for missing details. But I was not wrong.
BTW, one year is the maximum you can get from TN status (not VISA, as explained in fine detail by Xiaomimi above). You can get shorter duration if that's what's being offered.

If you got your last degree (the highest level) in China, or in Europe (say, Germany, a non-English spoken country), you'd better have it evaluated by a government recognized agency/organization. You will need it in a long run.
Though I have my diploma evaluated in Canada, my employer even asked me to have it re-evaluated by a US government certified agency.

Regarding to your question on evaluating only the last degree, the answer is YES if your last degree is related to your job offer. Say you have a Master in CS, and a Ph.D. in chemistry, but your new job in USA is in IT, then you'd better have your Master degree in CS evaluated. Clear?
For the English-speaking country question, there is no clear cut . Depending on where you are going to and who the employer is, most of the places I know acknowledge diplomas from Britain only, but not other English-speaking countries, say Singapore.
Nevertheless, USA is USA. I heard stories that a Canadian diploma needs to be evaluated as well.
