just make him sign it ... probably will cost some money, like $50 ... why don't you get your doc to sign it? or dentist, or your church prist (if you have a church to go)

如果找不到符合申请表中符合条件的担保人,直接去办护照的地方(申请表中有地址),填另一张表(PPT 132),大概花$20,就可以了.

I did it for my PR card. YOu can find the legal help centre online. There are several places in Ottawa. I remember the place that I got lawyer's signature is close to Summerset and Bank street.
