我现在已经停止工作了，ROE昨天也拿到了，我听说去办EI之前要下载一个申请表，不知道哪位姐妹可以帮我找到那个网址，十分感谢！

申请maternity and parental EI 只能在网上申请,如果你有问题,可以去 service Canada centers去问. 好像没有什么特殊的表格要下载的.
