Israel: Over 12,000 people test positive for COVID-19 after receiving Pfizer vaccine​

More than 12,400 people have tested positive for coronavirus in Israel after getting inoculated by the Pfizer/BioNtech vaccine, including 69 people who had taken their second jab.

Israel's health ministry tested 189,000 people after they vaccinated by Pfizer's vaccine and 6.6 per cent of the people still tested positive for COVID-19.

Earlier, Nachman Ash, national coordinator on the pandemic had said that Pfizer's vaccine was "less effective than we had thought".

Israel launched a vaccination drive on December 19 with a focus on the elderly, those with risky medical conditions and some emergency workers. More than a quarter of its citizens have now received the Pfizer Inc. vaccine, health officials say.

About a quarter of Israelis have received their first vaccine shot and 3.5 per cent already got their second dose.

Since the rollout of vaccinations one month ago, the Jewish state had inoculated more than 2.2 million of its nine million inhabitants, Health Minister Yuli Edelstein said Tuesday.
Still, the country is in its third lockdown with infection rates remaining high. More than half a million cases have been reported and 4,005 people have died in Israel since the pandemic began.

The rise in infections, despite the lockdown and vaccinations, has been attributed to virus mutations and disregard by some people of restrictions on assembly.

In return for rapid delivery of the Pfizer-BioNTech vaccine, Israel has committed to sharing with the US-German manufacturer its data on the effects of the mass immunisation.

Israel's vaccination programme is showing signs of working to drive down infections and illness in the over-60s.

The fall appears to be most pronounced in older people and areas furthest ahead in their immunisation efforts.

This suggests it is the vaccine, and not just the country's current lockdown, taking effect.

Israeli Ministry of Health (MoH) figures show 531 over-60s, out of almost 750,000 fully vaccinated, tested positive for coronavirus (0.07%).

And far fewer fell ill, with 38 becoming hospitalised with moderate, severe or critical disease - a tiny proportion.

The MoH assessed the medical records of almost a million people in total - 743,845 of whom were over the age of 60 - until at least seven days after they received a second dose of the vaccine.

There were three deaths in vaccinated over-60s - although it is possible they contracted the infection earlier, before their immunity had time to build.

Before the vaccine had time to take effect, more than 7,000 infections were recorded, just under 700 cases of moderate to critical illness and 307 deaths.

The MoH data suggests infections and illnesses fell consistently from 14 days after receiving the first jab onwards.

Delayed dose strategy​

While the biggest fall observed was in people seven days after their second dose of the vaccine, this cannot be used as evidence for or against the wisdom of the UK's strategy of delaying second doses.

Israel has consistently given people their first and second doses no more than three weeks apart, but the first dose isn't expected to provide protection for at least two weeks.

So it is not possible by looking at this data to untangle the impact of the second dose from the first dose starting to kick in.

The first dose takes longer to show any effect as it primes the immune system to start recognising and fighting off the virus.

Israel has delivered five million doses of the vaccine to a population of about nine million - and about one million people have received two doses.

The jury's still out on whether the vaccine completely stops people from carrying, and passing on, the virus.

So for now, while many people remain unvaccinated, those who have had the jab are still being told to socially distance and wear masks.

Lockdown a factor?​

The MoH's data only looked at infections and illness in vaccinated people - it did not involve a control group of unvaccinated people as a clinical trial would.

But a team of researchers at the Weizmann Institute of Science, Tel-Aviv University, and Technion - Israel Institute of Technology - have attempted to pin down whether the observed fall in cases is down to the vaccine rather than just the effect of lockdown restrictions.

Prof Malka Gorfine, Hagai Rossman, Prof Eran Segal, and Dr Uri Shalit analysed national data by age and city.

They identified bigger falls in infections and hospitalisations both in the over-60s, who were vaccinated first, and in the cities which vaccinated the largest proportion of their populations earliest.

These relative changes were not seen in earlier lockdowns.

This gives early but encouraging signs the falls can be linked to the vaccine, and not just to current restrictions.

Separately, Israel's second-largest healthcare provider Maccabi published figures showing 66 people out of a group of 248,000 vaccinated (0.03%) caught the Covid-19 virus, more than a week after receiving the second dose of the vaccine.

Of those 66, all had mild symptoms and none were hospitalised.

Unlike the MoH, Maccabi compared the infection rate with 900,000 unvaccinated people with a similar demographic profile. In this group during the same time period, 8,250 caught Covid - 11 times as many.

They estimated this meant the vaccine was 92% effective when used in a population - very similar to the 95% Pfizer found in a controlled clinical trial.

这个是蛮好的超大人群全民的试验。两个国家正好使用了不同的疫苗。

Israel: pfizer mRNA vaccine, vaccination % 55%

UAE sinopharm vaccine, vaccination % 33%

是目前第一名和第二名接种率都很高55% VS 33%。 很快就就会到达》70%全民接种率。那么国家的感染人数,感染率变化,住院率,死亡率的数据是非常有力的疫苗安全性和有效性的数据。不足是两个国家感染检测的力度,覆盖率,准确性,临床数据,新冠死亡人数的标准肯定是不可能一样的。这样就不是真正的比较。

但是有数据总比没有数据更好。

近来,世界疫情开始转好,患病人数虽然依然徘徊在高位,但感染人数,住院人数,重症人数都普遍下降。人们将此归功于疫苗,虽然被本国的媒体骂得狠,中国的媒体抹黑得紧,但美国接种的数据还是笑傲全球的,根据CDC的数据,美国迄今已经以每天134万人的速度接种了3200万剂疫苗,,美国感染人数也下降了三分之二。以美国强大的工业能力和组织能力,完成在三个月内接种一亿人,年内完成全民的接种,是完全可以预期的。

疫苗接种是一会事,效果如何是另一会事。由于对疫苗非暴露性的临床试验的结果持怀疑态度的人很多,包括发表在顶级科学期刊的文章,因此,实际接种后的效果,成了人们观察疫苗效果更好的指标。

当地时间1月25日,以色列四大健康保险服务机构之一的Maccabi表示,对12.86万名接种者接种两剂由辉瑞和德国生物技术共同研发的新冠疫苗一周后的状态进行确认,结果发现仅20人感染了新冠肺炎,感染率为0.015%。该调查并非医学上的临床研究,没有直接的对照组,但这是全球首个以完成两剂疫苗接种的普通人为对象进行的感染率调查,因此意义重大。按此结果计算的保护率约为92%,与辉瑞疫苗临床试验结果类似。但该结果来自于超过10万的大标本,可信度更大。以色列的每日确诊病例也由五千下降到三千余例,传染指数R下降到0.9以下。

但到了2月2日,确诊病例又升至8271例,9.2%的待检者核酸阳性。R值升至0.97%。而这时,以色列900万人口已有约35%施打辉瑞所建议两剂疫苗中的第1剂;另有约180万人打过第2剂,比1月25日的数值增加了10%。本来效果应该比25日更好,但实际上恶化了,这是怎么会事?

以色列国家抗疫专家小组主席巴雷克(Ran Balicer)表示,区分疫苗的两种成效非常重要,首先是“直接效果”,即指已接种民众能够“预防出现症状性疾病与重症”,其次是“间接效果”,即指疫苗能对足够多的人诱发足够的免疫力,以形成一种“流行病的阻绝”,防止病毒扩散 ,就是群体免疫。

以色列的数据显示,疫苗施打肯定减少了重症病患,但群体免疫的问题仍未有定论。Pfizer和Moderna早就说过,它们并没有证据显示其疫苗可以防止感染和继续传染,在临床试验中也没有加入(或无法)这项观察。

我的理解是这样的:大多数抗体都是血溶性的,如IgM,IgG, 只能对付血循环里的新冠病毒。有一种IgA,是分泌于粘膜粘液中的,可以结合在呼吸道,消化道表面的病毒。Pfizer和Moderna都这么表态,说明它们的疫苗无法产生有效IgA。强调一点,关于这个理论,还是有争议的。

因此,当一个人打了疫苗产生免疫后,再吸进病毒,由于在呼吸道表面没有IgA的保护,病毒仍然能够进入体内,因此公司说疫苗不能保护被传染。病毒进入血液后,会遭到抗体阻击,但保留在呼吸道粘膜细胞内的病毒,仍然可以生存,甚至繁殖。此人呼出气体中,仍然可能携带病毒,继续传染给别人,所以公司说不能防止传染。

如此,疫苗的作用,可能只是防止大量在呼吸道粘膜内繁殖产生的病毒进入血循环,到各组织产生伤害 从而出现中重度症状。但病毒仍然可能感染接种过疫苗的病人,也可以通过这个病人传染给别人。这个病人将是轻症或无症状感染者。那是不是这个疫苗就没用了呢?非也!病毒产生毒害,是通过血液传到全身后引起的,血液中的抗体能大量消灭病毒,因此打了疫苗后,发生症状的可能性减低了。而且,少量的粘膜细胞,并不能繁殖大量的病毒,慢慢的病毒就自然死亡了。

其实不少疫苗也是这样作用的,如破伤风疫苗,只是产生对破伤风杆菌分泌的毒素的抗体,只结合毒素使得其不产生症状,并不杀死杆菌。这样的疫苗,不会形成群体免疫,但还是有用的。

如果是这样的话,可能新冠将永远和我们在一起了,不会像天花一样绝种。而疫苗接种也需要100%,没接种的人,就有可能得上致命的重症,虽然只有5%的机会。

病毒的变种,还可能使得现有的疫苗失效,连那点保护重症的作用都消失了。

以色列卫生官员非常不愿意得出这样的结论,他们坚持出现反弹的原因是新冠新变种引起的疫苗效果减弱,以及由于传染能力提高而要求达到群体免疫的人口比例提高,不再是原来计算的70%,而是90%。由于原来16岁以下不打疫苗,那占了人口的30%,如果不打就达不到人口90%的指标。现在以色列提议将疫苗适用范围扩大到16岁以下人群,但按法规这需要临床试验的支持,可能不是一会半会能做的事。

总之,以色列出现的这一波折,让我们迅速得胜的希望破灭了,新冠是个比想象中更狡猾,更难对付的敌人。未雨绸缪,现在就应该为最坏的结果做准备了:准备好对全部人口而不是大部分人口接种,准备对付病毒变种的新疫苗,研究可以防止传染的新疫苗,研究可以杀灭或抑制病毒的药物(就像HIV一样,无法制作疫苗但可以制作药物)。

看来,征服新冠,任重而道远!

All Israelis from 16 years up now eligible for Covid-19 vaccination​

All Israelis aged 16 and over are eligible to receive a coronavirus vaccine, Health Minister Yuli Edelstein said Thursday.

“Come in your masses. Take advantage of a situation which exists in almost no other country in the world. This is the only way we will beat corona -- together,” the minister said in a statement.

Israel has rapidly rolled out its vaccination campaign, and the government aims to inoculate the entire country by the end of March.

3.3 million people in Israel have received a first dose of the vaccine, of which more than 1.9 million have also received their second shot, figures from the Israeli health ministry show. Israel’s population is about 9 million.

As Israel’s government considers whether to further extend the lockdown from tomorrow, new case numbers remain high.

Some 7,397 new cases were reported Wednesday, with a positivity rate on tests of 8.9%. The number of fatalities stands at 4,975.
